The Sullivan Centre provides education for children and young people who cannot attend their usual school because of their medical or mental health needs.

Referal Process – Sullivan Classrooms

The Sullivan Centre is a referral led provision that supports young people who are unable to attend school due to medical or mental health reasons.

Formal admission requests to The Sullivan Centre classrooms can be made via the following referral pathways:

  • From schools, all referrals must include supporting evidence from a medical professional e.g. Consultant paediatrician or CAMHs clinical nurse/psychologist) in line with the Health and Medical Needs Policy co-constructed with the Local Authority.
  • Directly from a Consultant or CAMHs when a child is enrolled at a school;
  • Through coordination with the Hospital Education Service (located at Hull Royal Infirmary) and Inspire inpatient unit.

A child with an Education, Health & Care Plan (EHCP) must also have support from the SEND team.

Direct referrals cannot be received from parents/carers and will only be considered in joint partnerships with a child’s dual registered School and other health care professionals supporting the child.

All referrals need to be signed by those with parental responsibility.

How to make a referral

To make a referral for a place at The Sullivan Centre classrooms please complete the referral form and ensure you include supportive evidence for this placement from a medical/health professional .

All new referrals made by schools and health professionals will be considered by The Sullivan Centre Admissions Panel which meets on a 4 weekly basis.

Our admissions panel constitutes; Senior Leaders within Sullivan Centre and representatives from CAMHS, Speech & Language Service, the Local Authority, Specialist Nursing Team & Educational Psychology.
